DOHMH Corona Health Center. (that's where it was for me)
On the appointed date and time, you talk with the person from the
DOHMH Corona Health Center.

(for me it was from 3/09/2020、26 weeks）

Pay from January to March in 2020. (I answered with my pay per week)
Then you choose the insurance company over the phone and they will tell you the monthly fee. (In my case, Fidelis Care's Medical with Dental and Vision was a monthly fee of $31.14 and I was able to join from 05/01/2020 - 04/30/2021)
